Concerns at hospital
Marjorie Tortice, 84, was admitted to the hospital after a fall on May 1 last year, and died at 4am on May 23.
Recording a narrative verdict at Mrs Tortice’s inquest at Bradford Coroner’s Court on Tuesday, Dr Dominic Bell said: “She received appropriate surgical management of her injuries.

Hide Ad“However, there were significant failures in documentation and communication between the medical, pharmacy and nursing teams.”
Dr Bell said treatment that might have prevented Mrs Tortice, of Newlands Drive, Morley, dying when she did, was not given.
After the inquest, Professor Tim Hendra, medical director at The Mid Yorkshire Hospitals NHS Trust, said: “On behalf of the Trust, I would like to extend our sincere sympathies to Mrs Tortice’s family.
“We recognise that some aspects of care fell below the expected standard and I apologise for this.”
He said an investigation had been carried out and improvements had been made.
