North Kirklees Clinical Commissioning Group is working with Kirklees Council and local service providers to transform the way community services are offered.

The aim is to reduce the amount of time patients spend in hospital, offer more care closer to home and make sure services work closer together to give patients the best possible experience.

CCG head of transformation Helen Severns visited Dewsbury Area Committee on Tuesday to talk about the progress to date.

She said: “Everybody has a bed in this health and social care community and that bed is at home.

“The evidence is people progress and improve sooner with care in their own home. With that we also need to ensure we’re supporting carers.”

The three key areas being looked at are proactive care to prevent people needing to go into hospital, crisis intervention planning and supporting earlier hospital discharges.

Several pilots of schemes to support the aims are under way or due to begin soon.

They include extended hours for certain Locala Community Partnerships services and stationing a continuing care nurse at Dewsbury and District Hospital.

The CCG also gave a presentation about its plans at Spen Valley Area Committee last night (Thursday).

The presentation included a video on integrated health care produced by independent health charity The Kings Fund.
